The Posture Myth and What Actually Predicts Neck Pain
Studies repeatedly fail to find the strong link between everyday posture and neck pain that clinical folklore assumes. Load tolerance and variability look more important.

What the cohort data show
Studies measuring habitual head and neck posture — including adolescent cohorts with objectively measured screen posture — have generally failed to find the strong prospective association with neck pain that the popular narrative predicts. Effects, where present, are small.
"Text neck" as a distinct pathological entity is not well supported. Neck discomfort after long screen sessions is real; attributing it to a specific spinal angle is where the reasoning breaks down.
A better-supported explanation
Sustained static loading of any posture appears more relevant than which posture it is. Muscles kept in one configuration for hours accumulate discomfort regardless of alignment. Variability — changing position frequently — is the intervention that follows from this, and it is easier to sustain than vigilance about alignment.
This is labelled Emerging Research: the negative evidence against strong posture claims is reasonably solid, while the positive case for variability as a treatment target is still being built.
- Change position roughly every 30 to 45 minutes
- Build neck and shoulder load capacity rather than chasing alignment
- Treat prolonged static work as the exposure, not the angle
Common questions
- So ergonomics doesn't matter?
- It matters for comfort and for making movement easier. The claim we question is narrower: that a specific posture causes chronic neck pain in otherwise healthy people.
